Information Collection, Use, and Sharing
We are the sole owners of the information collected on this site. We only have access to/collect information that you voluntarily give us via email or other direct contact from you. We will not sell or rent this information to anyone.
We will use your information to respond to you, regarding the reason you contacted us. Your information will only be stored in our business support systems. We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request (e.g., to connect you with a service or employer).
Unless you ask us not to, we may contact you via email in the future to tell you about the company, our affiliates, our services or changes to this privacy policy.
Your Access to and Control Over Information
You may opt out of any future contacts from us at any time. You can do the following at any time by contacting us via the email address or phone number given on our website:
- See what data we have about you, if any.
- Change/correct any data we have about you.
- Have us delete any data we have about you.
- Express any concern you have about our use of your data.
Security
We take precautions to protect your information. When you submit sensitive information via the website, your information is protected both online and offline.
Wherever we collect sensitive information (such as credit card data), that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a closed lock icon at the bottom of your web browser, or looking for "https" at the beginning of the address of the web page.
While we use encryption to protect sensitive information transmitted online, we also protect your information offline. Only employees who need the information to perform a specific job (for example, billing or customer service) are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are kept in a secure environment.

Pokket Privacy Policy
This Privacy Policy describes how we, Acivilate, Inc., collect personal information and how we use it. It covers our website (including the web-based POKKET application located at mypokket.com) and any mobile applications that we offer. We refer to both our website and these applications as our “platform.”
Summary
This section is a summary of our Privacy Policy. Please review the entire Privacy Policy that follows this summary for a complete description of how we collect and use personal data.
Our intent is to share your information only to help you and other people like you who register with us to use the platform (in this Privacy Policy, we refer to such individuals as “Users”), and agencies and other organizations that also want to help you or people like you, such as health care providers, relocation counselors, social service agencies, and the like (“Service Providers”).
Our platform is designed to improve communication and collaboration among “Sponsors” (such as community supervision organizations and state and local government organizations), Service Providers, reentering citizens and their families. Because our platform handles sensitive and legally-protected personal records, we have designed it to place Users in control of the sharing and use of their own personal information.
We collect personal information from you when you enter it into the platform. Sponsors, Service Providers, and others may also upload information about you to our platform.
In general, we will not share your personal information with any outside party such as a Service Provider except when you ask us to do so. For example, you may voluntarily release personal information through our platform to Service Providers that are consulting with you.
There are certain situations where we may be required to, or otherwise will, share your personal information including, for example, if we receive a court order, subpoena or other legal process or if we transfer our business to another company. We may also share anonymous, statistical data that do not identify you, with third parties.
If you sign up for one of our email lists, we may send you updates according to our email policy below.
You have certain responsibilities with regard to the privacy and security of your information, for example, to not share information that may be harmful to you and others, to protect the privacy of your information, and to let us know of improper use of the platform (for example, by children under age 13).

Information We Collect
Personal Information
When you register with us for access to the platform (whether the website or an application), and successfully complete the account registration process, you will become a “User” of the platform. As a User, you will have the opportunity to create a profile (“profile”) and to add information, data, and “Contacts” (people or organizations that can use the platform to contribute and receive information about you, including Service Providers and Sponsors) to the platform. To use the platform, you will need to provide certain personal information such as your real name and contact information, and you and others may provide additional personal information.
“Personal information” is information that may be used to identify you as an individual or that is defined as personal information under applicable law. Examples of personal information used in our platform include, among others:
- Your name
- Your contact information (address, phone number, etc.)
- Documents, images, and other files about you that you or other users upload to the platform
- Identifying numbers (such as a social security number or state identity number)
- Location information under your control (see below).
Certain other information will also be considered “personal information” if it contains any of the above or if it could be used by itself or combined with other information to identify an individual. For example:
- Employment information
- Education information
- Contacts that you may make through the platform (for example, with case workers)
- Other plans and records unique to you, such as reentry plans or treatment plans and your completion of plan tasks
- Medical information including receipts from counseling appointments
- Criminal history records information
- Identifying documents such as a copy of your social security card
We collect personal information from you when you voluntarily provide it to us, such as by completing a form or submitting a record on the platform. We also receive personal information about you from Contacts that you interact with through the platform such as Service Providers. You can view and manage this third-party information through the platform. We will also provide you with copies of any communication about you between Service Providers on our platform.
If you choose to provide us with a third party's personal information (such as name, email or phone number), you represent that you have the third party's permission to do so. Examples include providing the contact information for a mentor or counselor or a family member. We are not responsible for information that you or another user or third party provides. Any information that you provide to Contacts on the platform may be further disclosed or used, and we have no control over such further use or disclosure. See “Risks of Disclosure” below.
Location Information
With your consent, the platform may also collect information from your mobile device about your current location at a given point in time. For example, you may voluntarily “check in” to confirm your attendance at an appointment. You may enable or disable this collection (see “Location Data Preferences” below).
Analytics
As you use the platform, the servers that operate the platform automatically log certain technical and other anonymous information such as:
- the URLs accessed on our website or features used in our mobile applications;
- the IP address of your computer or mobile device connecting to the platform;
- the Internet hostname associated with that IP address; and
- the types and versions of your web browser and computer or mobile device operating system.
The platform may also place “cookies” or other persistent identifiers on your computer or mobile device. Cookies are small files that let the platform identify your computer or mobile device on subsequent visits. The platform may use cookies to store data about you between visits such as your preferences or login information. You can configure your browser to reject cookies from the platform. Please note that certain features of the platform may not be available if your browser does not accept cookies from the platform.
Additionally, your browser may offer you a “Do Not Track” option, which allows you to signal to operators of websites and web applications and services that you do not wish such operators to track certain of your online activities over time and across different websites. Our website does not respond to Do Not Track requests at this time, but we do not individually track the actions of individuals who are not logged in. If a User is logged into the service, we provide services based on the information provided through Pokket, but not through cookies or other means from any other websites. Therefore we consider we comply with the intent of Do Not Track. We do not collect online activity after you leave our Site.
The platform may also provide analysis to help improve the services provided to Users and Service Providers. Examples might include:
- personalizing recommendations for Service Providers and services;
- identifying how many Users do not have adequate housing;
- determining, on average, how long it takes Users to find employment; and
- understanding the attributes of Users who find it difficult to get their specific needs met.
Aggregated Information
In addition to the information described under “Analytics” above, we may generate aggregated information or statistical information from personal data after removing any information that can be used to identify you or another individual. Additionally, if we prepare aggregated or statistical information to share with non-Users, we will remove any information that can be used to identify a particular Service Provider or Sponsor as the source of this information unless the Service Provider or Sponsor explicitly provides its approval.
How We Use Your Information
General
We use your personal information and anonymous information to operate the platform. This includes making your records available to you and to your Contacts, connecting you with other Contacts, and storing your information for future visits.
We also use anonymous information to monitor and improve the functioning of the platform and our services as well as for other purposes. For example, we may use your IP address to diagnose a network connection issue or use anonymous usage data to understand how to improve our user interface, or to help a community understand the needs of the community.
Location Information
If you choose to provide location information to us, such as in response to a request to “check in,” we will share that information with the person making the request. This location information will also become part of your record on our platform and will be available to others, including Service Providers and Sponsors, who you permit to access your information.
E-mail Updates
You may also sign up to our e-mail newsletters using the platform. These newsletters periodically provide news and updates about the platform to members of these lists. We will not send you these newsletters unless you opt in to receive them and you may opt out at any time (see “E-mail Preferences” below).
We may also e-mail you from time to time with messages about your account, including transaction confirmations.
Information Sharing
We will not disclose your personal information to third parties except:
- where you have expressly authorized us to share that information with the third party through the platform (such as Service Providers, Sponsors, or family members);
- with our third-party hosting and other vendors where necessary to operate the platform under terms that restrict further disclosure of the information;
- in connection with a merger, acquisition, or transfer of our business or assets to another entity;
- where we may be required by law, court order or other legal process to disclose your information; or
- other situations as described in this Privacy Policy.
Unless we are restricted from doing so, we will notify you of any request or order to disclose your information and provide you with an opportunity to intervene. Certain types of information are protected by law from disclosure such as attorney-client communication, therapist-client communication, alcohol and drug abuse treatment records, and certain medical records. Providing certain information to the platform that would otherwise be confidential may result in losing or waiving such confidentiality, and providing information to the platform that would otherwise be protected from re-disclosure may result in that information being re-disclosed. If you do not want to lose these protections or this confidentiality, be careful not to place on or disclose through the platform information that is private or inappropriate.
You may choose to authorize us in advance to share your personal information with approved health care providers in case of a medical emergency, such as where your life is at risk. The identity of those parties, where applicable, will be disclosed when you sign up for services.
In certain, unusual circumstances, we may use or disclose information, including personal information, about you or another person without your consent in order to protect against potential harm to one or more people. For example:
if you have provided medical information to us that may be important to your health, and you are unable or unwilling to communicate such information, we may share it with a health care provider in an urgent situation; or
we may share information that reveals a serious and imminent threat to you or others with a person who is reasonably able to prevent or lessen the threat.
Risks of Disclosure
Self-incrimination
We are not responsible for the consequences of you choosing to use the platform or to disclose information through the platform. You use the platform at your own risk. For example, if you choose to provide certain location or other information, such information could be used by those you have authorized to see the information. If the information suggests inappropriate or illegal activity, it could be used as evidence against you in an administrative, civil or criminal proceeding. If you share alcohol or drug abuse treatment information from a drug or alcohol abuse treatment provider through the platform, that information may be used to criminally investigate and prosecute you, even though that information could not be used for these purposes if other users of the platform received the information by other means. The information you provide to us may be disclosed, at your option, to governmental agencies and other Sponsors for purposes of monitoring your activities. If you do not wish certain information to be shared, you should either not provide such information or limit who may access such information. As noted above, providing certain information may also make such information subject to further disclosure and/or waive certain rights (such as the attorney-client or other legal privilege).
Violation of Rights, Suit, Inadmissibility of Evidence
Likewise, we are not responsible for the effect of any requests for collection of, access to or receipt of information by Service Providers, Users or others. For example, if a governmental agency or counsel should request information in violation of constitutional, statutory or other rights, we are not responsible for such request, the disclosure of information (through the platform or otherwise), or the effect of such request or disclosure. Please note that the improper request for, or receipt of, information may result in civil or criminal suit, the inadmissibility of certain evidence and/or sanctions again the receiver of the information.
Your Choices
General
If personal information you have submitted through the platform is no longer accurate, current, or complete, you may update it yourself on the website. If you are unable to do so, please contact us at support@acivilate.com. Upon appropriate request we will usually be glad to update or amend your information, but we reserve the right to use information obtained previously to verify your identity or take other actions that we believe are appropriate and lawful.
E-mail Preferences
You may opt-out of receiving any or all of our newsletters by following the opt-out instructions contained in those communications. Those communications may also inform you how to update your personal information related to such communications.
Location Data Preferences
Your mobile device may allow you to disable collection of location information. Please note that if you do not allow our mobile application to collect location information, access to some features of the application will be disabled.
Push Notifications
Our mobile application or platform can be configured to display push notifications. These are alerts that display on your mobile device. For example, you may receive an alert that there is a message for you on the platform. You may use your device’s settings to enable or disable these notifications. We are not responsible for consequences that arise based on your decision to enable or disable particular notifications.
Security
We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. We use industry-standard encryption to protect personal information in transit between your computer or mobile device and our platform and at rest (stored) on our platform.
The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to the platform, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. You are also responsible for data maintained on your computer or mobile device, as we cannot protect such data. Likewise, be careful in choosing with whom you share your information through the platform. Use caution in allowing others to access your computer or device.
Unfortunately, the transmission of information via the Internet and mobile platforms cannot be made completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted through or resting on our platform.
Privacy and Security Laws
Information collected, received, used, accessed or disclosed within the platform is received and disclosed solely with the authorization of Users. Except with the specific consent of Acivilate, Inc., Acivilate is not acting on behalf of a health care provider, health care payer, clearinghouse or any other “covered entity” under state or federal privacy and/or security laws and is not a “business associate” as that term is defined in the Health Insurance Portability and Accountability Act (“HIPAA”). Covered entities utilizing the platform are solely responsible for compliance with HIPAA, and Acivilate does not consent to act on behalf of any covered entity accessing, using, maintaining, or disclosing personal information using the platform.
Retention of Personal Information
We will retain your personal information as needed to fulfill the purposes for which it was collected. We will retain and use your personal information as necessary to comply with our business requirements and legal obligations, and to resolve disputes, protect our assets, and enforce our agreements.
Children's Privacy
We do not knowingly collect or solicit personal information from children under the age of 13. By using the platform, you represent that you are at least 13 years old. If you are under 13, please do not use the website or any application or attempt to register for any services or send any personal information about yourself to us. If you are considered a minor by your state, please do not attempt to download, register for, or create a profile. If we become aware that we have inadvertently received or collected personal information from an individual who is under the age of 13, we will attempt to immediately delete that information from our files and records. Furthermore, we encourage Users who are between the ages of 13 and 18 years old to ask their parents or guardians for permission before sending any information about themselves over the Internet.

Where Information is Maintained
By using the platform, you agree that the collection, use, transfer, and disclosure of your personal information and communications will be governed by the applicable laws in the United States.
Electronic Communications
By using our services, or otherwise providing personal information to us, you agree that we may communicate with you electronically regarding security, privacy, and administrative issues relating to your use. For example, if we learn of a security system's breach, we may attempt to notify you electronically by posting a notice on our websites, by sending an email, or otherwise contacting you.
Changes to this Policy
We may change this Policy at any time and from time to time by providing notice to you of any material changes. The amended version of this Policy will apply to your use of the service after we give that notice.
